Our care is personalized, one-on-one, and focused on helping you move better, feel stronger, and build lasting results. Because insurance models can place limitations on how care is delivered, Revive operates as an out-of-network (OON) provider for many insurance plans.

This allows us to focus on what matters most: your goals, your progress, and the care you need to get there.

Choosing out-of-network care allows us to provide a level of individualized attention that can be difficult to achieve within traditional insurance-based models.

With Revive, you can expect:

Depending on your insurance plan, you may have out-of-network benefits that reimburse a portion of your physical therapy expenses. We can provide you with a detailed superbill that contains the information needed to submit a claim to your insurance company.

Choose the Care That Fits Your Goals

At Revive, we recognize that your needs can change over time. Some patients need focused rehabilitation for an injury or surgery, while others want ongoing support to stay strong, active, and resilient.

That’s why we offer two pathways for care.

Physical Therapy — Plan of Care

For recovery, rehabilitation, and active concerns.

If you’re dealing with an injury, recovering from surgery, managing a chronic condition, or working through pain or movement limitations, your physical therapist will develop an individualized Plan of Care based on your specific needs and goals.

Your plan may include hands-on treatment, therapeutic exercise, neuromuscular training, movement retraining, education, and other evidence-based strategies as appropriate.

Rather than following a predetermined number of visits, we focus on what your body needs to make meaningful progress.

Physical Therapy Plans of Care are available in packages based on the number of sessions recommended by your physical therapist.

Your therapist will discuss your recommended plan with you so you understand what to expect and why each stage of care is important.

Continuity of Care

What happens after you get better?

Most people finish physical therapy, and then stop.

Progress can fade. Old movement patterns can return. Strength can decline. And the things that contributed to an injury in the first place can slowly creep back in.

At Revive, we believe there is another option.

Continuity of Care is designed to bridge the gap between rehabilitation and long-term wellness.

These one-on-one wellness training sessions are guided by our licensed physical therapist who already understands your body, your movement patterns, your history, and your goals.

This isn’t a generic workout.

It’s a personalized, clinically informed approach to staying strong, moving well, and continuing to build on the progress you’ve made.

Continuity of Care may be right for you if you:

  • Have completed a physical therapy Plan of Care at Revive and want ongoing support
  • Want to maintain the strength and mobility you’ve gained through physical therapy
  • Want to prevent future injuries or recurring pain
  • Are looking to improve athletic performance
  • Want guidance with strength training or exercise technique
  • Want to improve movement quality and form
  • Want support maintaining healthy movement as you age
  • Simply want a more individualized approach to wellness and fitness

What We Focus On

Your sessions may incorporate:

How the Membership Works

Continuity of Care is offered as a monthly membership, with a set number of sessions available each month based on your provider’s recommendations and your individual goals.

Unused sessions roll over to the following month, allowing your care to remain flexible as your schedule and needs change.

Your provider will recommend the membership level that best fits your goals, activity level, and desired frequency of care.
